If you are unable to keep your appointment, please call us as far in advance as possible. We make a sincere effort to adhere to our appointment schedule. At your first appointment, allow yourself an additional 30 minutes to register at the reception desk and complete any necessary paperwork. The forms you will need to fill out are available for download on this website below.
WHAT TO BRING
Medical Records
If possible, please bring any x-rays, test results, or pertinent medical records to the office at the time of the visit. If necessary, outside medical records or test results can be faxed to 561-733-5851
Personal Medical History
In order to provide the best care, Dr. Wolfson needs specific information about the medications you are taking and the details of your current injury. Please bring a record of all current prescription and non prescription medications. Specific information about your current injury and any related history is also very helpful.
EMERGENCIES
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Personalized Orthopedics of the Palm Beaches, and they will contact us.
If you have an urgent medical need and need to talk to our providers after our regular hours, please call our office at (561) 733-5888. A doctor is on-call 24 hours a day. Your call will be returned promptly.

Insurance
At Personalized Orthopedics of the Palm Beaches we accept workers’ compensation, Medicare, and most major insurance plans. Please contact your insurance company or you may call our office at (561) 733-5888 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa and MasterCard.

We accept many forms of payment including cash, money order, as well as Visa, Master Card, Discover, and American Express.
